853 of the 3,755 listings in this directory record which waivers they accept. A provider missing from this list has not told us either way — it does not mean they refuse the waiver. Confirm funding directly with the provider and with your lead agency before you rely on it.
What Alternative Care (AC) covers
Alternative Care funds a similar set of home and community-based services to the Elderly Waiver for people who would otherwise pay privately. DHS publishes the rate limits it will pay for each service.
Eligibility is decided by a MnCHOICES assessment at your county or Tribal lead agency, not by a provider and not by this directory. Free statewide help: Disability Hub MN on 1-866-333-2466 for people under 65, Minnesota Aging Pathways on 1-800-333-2433 for 65 and over.
