How someone gets a waiver in Minnesota

  1. Start with the lead agency. There is no statewide central intake — eligibility runs through a MnCHOICES assessment by a certified assessor at your county or Tribal lead agency.
  2. The assessment sets the level of care. Which programme someone qualifies for follows from that assessment and from Medical Assistance eligibility, not from anything a provider or this directory can decide.
  3. Free help exists. Disability Hub MN (1-866-333-2466) advises people under 65; Minnesota Aging Pathways (1-800-333-2433) covers 65 and over. Neither sells anything.
