How someone gets a waiver in Minnesota
- Start with the lead agency. There is no statewide central intake — eligibility runs through a MnCHOICES assessment by a certified assessor at your county or Tribal lead agency.
- The assessment sets the level of care. Which programme someone qualifies for follows from that assessment and from Medical Assistance eligibility, not from anything a provider or this directory can decide.
- Free help exists. Disability Hub MN (1-866-333-2466) advises people under 65; Minnesota Aging Pathways (1-800-333-2433) covers 65 and over. Neither sells anything.
