What CADI Waiver covers
CADI — Community Access for Disability Inclusion — is one of four Minnesota waivers serving people under 65 with disabilities, alongside DD, BI and CAC. Each requires Medical Assistance eligibility and an assessment through your county or Tribal lead agency, and each funds a different level of care.

Eligibility is decided by a MnCHOICES assessment at your county or Tribal lead agency, not by a provider and not by this directory. Free statewide help: Disability Hub MN on 1-866-333-2466 for people under 65, Minnesota Aging Pathways on 1-800-333-2433 for 65 and over.
